Photo ID Cards - Team Officials

All CMSA team officials for U10 – U19 teams require a valid CMSA Photo ID card. This card must be present at the start of each game so that the referee can identify the coaches on the field.

Team official cards are valid for three (3) years. The cost for new cards or replacement cards is $10.00. If you have an old card that has expired, there is no cost for a new one if you bring in your expired card.

You must register with a club and listed on a roster before we are able to process your ID card. 

Reminder:  No Team official cards will be printed or re-issued unless coach training requirements are met. Team officials are strongly encouraged to begin completing their requirements early to avoid delays in receiving their Photo ID cards. If you are unable to bring your child with you, please upload their photo to Demosphere before arriving at the CMSA office. Players do not need to be present to receive a Photo ID as long as their photo has been uploaded. If you are unable to upload the photo, you may bring your child to the CMSA office and we will take the photo for you.
