Submissions
Travel Permit Applications can be found on the linked Alberta Soccer website. Handwritten rosters and travel permits will not be accepted.
Completed forms may be submitted in person, faxed to 403-236-3669 or scanned/e-mailed to ekalac@calgaryminorsoccer.com. Payment can be made in person by cash, cheque, Visa, MasterCard, debit card or over the phone during regular office hours by Visa or MasterCard.

Fees
Travel Outside Alberta, Within Canada
For travel outside of Alberta (within Canada) please submit a Travel Permit request and team roster to the CMSA office a minimum of TWO weeks prior to departure.
CMSA Fee: $15
Canada Soccer and Alberta Soccer Fee: CLICK HERE
** CMSA will charge a $50 late fee for all travel permits submitted less than 2 week prior to travel.
Travel Within The United States
For travel to the United States, please submit a Travel Permit request, team roster, mandatory travel insurance registration form to the CMSA office a minimum of FIVE weeks prior to departure.
CMSA: $20
Canada Soccer and Alberta Soccer Fee: CLICK HERE
** CMSA will charge a $50 late fee for all travel permits within the USA submitted 1 week or less prior to travel.
International Travel
For travel to the Internationally, please submit a Travel Permit request, team roster, mandatory travel insurance registration form to the CMSA office a minimum of EIGHT weeks prior to departure.
